Job Summary

Under general supervision, provides goal directed Speech and Language Pathology evaluation, treatment, and discharge planning. Delivers safe and competent speech therapy as ordered by a physician. Provides initial and ongoing assessment of patients. Evaluates patient's abilities and limitations to develop a therapeutic plan for patients with speech, language or cognitive disorders.

Essential Functions: General Operations: Completes a thorough evaluation based on presenting diagnosis and pertinent information regarding the patient. All evaluations must conform to departmental, hospital, and professional standards. Develops goals for treatment appropriately based on specific problems identified by evaluation. Goals are objective, measurable, reasonable, and described in functional or behavioral terms. A time frame for goal achievement is stated. Goals and treatment plan are coordinated with the interdisciplinary treatment team. Receives authorization prior to treatment as stipulated per insurance carrier. Monthly productivity meets the minimum stated departmental requirements. Charges are accurate and completed on the date of service. Makes sure charges reflect the actual time spent with the patient. Statistics are completed at the end of each day. Utilizes support staff effectively and appropriately as related to patient care and office duties. Provides constructive feedback and instruction to support staff in relation to patient care. Assists in orientation/training of new employees, hospital staff, and Speech students during orientation and as assigned. (20%) Treatment: Inpatients are re-evaluated at least every two weeks. Outpatients are re-evaluated monthly, unless participation in specific programs indicates otherwise. Modifies treatment plan with re-evaluation or changes in patient status and coincides with goal and plan. Performs treatment within accepted professional guidelines. The treatment used is appropriate to the diagnosis and is modified to meet the changing needs or changing response to therapeutic intervention. (20%) Discharge: Bases discharge on goal achievement, change in medical status, or departmental policy. Provides discharge instructions to patient/family/significant other appropriate for age, and learning ability, need for adaptive equipment is identified and addressed. Completes discharge summary and makes recommendations for follow-up are made. (20%) Communication: Patient and family/significant other are involved in goal setting where appropriate. Behavior and communication are courteous and respectful. Maintains constructive working relationships with all staff through courtesy, dignity and respect. Reacts in a positive manner to questions, suggestions, and constructive criticism. Is tactful and considerate. Relays communication timely and to the appropriate person. (20%) Documentation: Completes patient care documentation per departmental guidelines. Updates and completes patient information to reflect patient's current clinical status daily. Data bases, evaluations, progress notes and discharge summaries are consistently kept current. Completes documentation and conference notes clearly and on the date of service. Meets the minimum stated departmental requirements for monthly productivity. (20%)

Education: Master's Degree required

Work Experience: 1-3 years experience in an acute care / rehabilitation setting equired

Licenses: BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association Required

Knowledge, Skills and Abilities: Knowledge of theory, principles and practices of Speech - Language Pathology.
