Job Description:

Position: Licensed Master's Social Worker

Company: Elara Caring Hospice

Location: Jackson County

Schedule: Monday through Friday

Responsibilities:

  • Ensure alignment of activities with Elara Caring's vision and leadership directives.

  • Assess patients to identify psychosocial, financial, and environmental needs.

  • Conduct initial social work evaluation visit and reevaluate patient's needs during subsequent visits.

  • Communicate significant findings and changes in condition to supervisor, physician, and relevant personnel.

  • Implement safety plans using patient, family, and community resources.

  • Participate in the development and implementation of the Plan of Care to ensure quality and continuity of care.

  • Verify Plan of Care before each visit and provide care according to physician's orders and established standards.

  • Initiate and revise Plan of Care as needed in response to patient care issues.

  • Write physician orders for additional visits or changes to the plan of care.

  • Counsel, instruct, and involve patients and families in following the Plan of Care and addressing social work-related needs.

Requirements:

  • Master's or Doctoral Degree in Social Work from a Council of Social Work Education accredited institution.

  • Minimum 1 year of social work experience in a healthcare setting.

  • Demonstrated ability in casework, counseling, and group work preferred.

  • Current State license as a Medical Social Worker.
