How you make a difference

MAT Discharge Planners are responsible for delivering individual and group treatment to identified MAT patients scheduled for community release, facilitating a seamless transition process. They coordinate, execute, and oversee discharge planning within correctional facilities, maintaining open lines of communication with both facility personnel and external community resources. This role necessitates proactive transition planning, commencing at least three months before the patient's release, and entails assisting individuals in accessing local resources and scheduling appointments with relevant programs or providers to ensure a successful transition to community treatment.

Key Responsibilities

  • Conduct discharge planning and individual/group treatment for MAT patients, adhering to facility policies and procedures, while establishing relationships with community agencies for mental health and medical services.

  • Deliver evidence-based group treatment focusing on relapse prevention and healthy living, employing techniques such as cognitive behavioral treatment, motivational interviewing, and trauma-informed therapies.

  • Facilitate eligibility applications for financial assistance programs like SSDI and SSI, as well as the resumption of benefits for inmates nearing release, while maintaining effective collaborations with facility staff, parole officers, and medical personnel.

  • Initiate contact with inmates requiring mental health or medical care at least 120 days before release, identify their release-planning needs, provide life skills programs, coordinate medication and medical needs post-release, and assist in scheduling appointments with mental health agencies.

  • Document all inmate services in health records, maintain necessary tracking logs for post-release follow-ups, participate in relevant meetings, training, and community-based activities, and ensure compliance with facility employee standards.

Qualifications & Requirements

Education

  • Master’s degree in social work or closely related field from an accredited college or university.

Experience

  • Previous experience in developing successful transition plans involving community resources for persons with mental illness.

  • Must also have experience in social work case management in a mental health agency for mentally ill population.

Licenses/Certifications

  • Must maintain CPR certification, and any other certifications (such as First Aid) required by contract.

  • Must be eligible for LMHC or LICSW licensure, and be eligible for CADC or LADC certification/licensure.

Additional Details

MILWAUKEE CO JAIL & CRC IN FRANKLIN

1.0 FTE Monday-Friday 8am-4:30pm. Position is shared between MKE Co Jail and MKE Co CRC. Preferred candidate has experience working in substance use treatment or medication assisted treatment programs. Also preferred experience with case management or social work experience in substance use or AODA. No licensure is required. Bachelors degree in psychology, social work or other related field required.
