Department of Human Services DHS/DAS Social Services Program Consultant Supervisor (TOC - Team Lead) - 00151938 in Cleveland, Georgia

Pay Grade: L

Supervises, guides, and/or instructs the work assignments of subordinate staff. Conducts research involving data collection and analysis, develops and implements policy, and administers social services programs.

Under the broad supervision of the Access to Services Section Manager, serves as Team Lead for the Transitions and Options Counseling Team. Supervises staff responsible for the Care Transitions (Money Follows the Person and Nursing Home Transitions) and Options Counseling programs.

Role and Responsibilities:

  • Serve as Team Lead for Care Transitions and Options Counseling programs.
  • Oversee all aspects of contract management for Transitions and Options Counseling programs.
  • Responsible for policy writing/revisions for Transitions and Options Counseling programs.
  • Serve as subject matter expert for Transitions and Options Counseling programs in the DAS Data System (DDS).
  • Appy for and manage grants related to Transitions and Options Counseling.
  • Build partnerships with other state agencies and organizations including the twelve Area Agencies on Aging, Centers for Independent Living, and Department of Community Health.
  • Ensure programs are monitored; provide feedback and technical assistance to contractors.
  • Work with the Program Integrity Section of DAS to monitor Care Transitions and Options Counseling data and ensure accurate and timely reporting to grantors, federal and state entities, and provide technical assistance.
  • Engage in required statewide travel and attend all required meetings/trainings related to care transitions and options counseling programs.
  • Serve as team liaison to standing meetings, including Leadership Team and AAA meetings.
  • Serve as an active team member for other program initiatives throughout the Division of Aging Services, including program workflows and other duties as assigned.
