Northwell Health Care Coordination Associate in Great Neck, New York

Job Description

Supports integrated operations and transitions of care activities. Coordinates patient placement with post-acute care facilities and/or services, transportation, and obtains insurance authorization to facilitate safe transitions of care. Communicates with all pertinent individuals or agencies to fulfil outstanding transition of care items identified in the patient's care plan. Performs assigned tasks to assist with quality care and patient throughput.

Job Responsibility

1.Follows standard policies, techniques, and practices to facilitate safe transitions of care, including but not limited to post-acute placement, transportation, and/or appointments, and obtain appropriate insurance authorization for the next level of care

2.Functions as liaison with all pertinent individuals or agencies that have an impact on patient transition of care

3.Uses team-based communication strategies to close the loop on referrals, hospital follow-ups, and any outstanding transition of care items identified in the patient's care plan

4.Performs responsibilities supporting the operations needs of Integrated Operations

5.Gathers and generates reports on various department metrics for review by management; recommends processes and procedures to improve departmental performance

6.Exercises judgement within defined procedures and practices to determine appropriate action. Delivers information and collaborates on problems/issues within defined parameters

7.Understands professional concepts/standards of own work area, regulations, strategies, and operating standards

8.Decisions are made within documented parameters and instruction around professional guidelines and compliance requirements

9.Follows direction. Work is closely managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation

10.Typically receives detailed instructions on work, approach, and priorities

11.Work approach may be reviewed by more senior team members, lead and/or management

Job Qualification

1.Bachelor's Degree required, or equivalent combination of education and related experience

2.0-1 years of relevant experience, required.

  1. Shift 9:30 am-5:30pm, includes rotating weekends.

The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.When determining a team member's base salary and/or rate, several factors may be considered as applicable (e.g., location, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget and internal equity).

The salary range for this position is $58,500-$79,300/year
