As a Registered Nurse, you'll perform all appropriate nursing assessments, evaluate clients' physical condition and service needs, coordinate care with the treatment team, and oversee medication administration and compliance. Key Responsibilities:

  • Expert Guidance: Provide expert advice to the medical director and clinical teams to support practice transformation within Patient-Centered Medical Home (PCMH) and Patient-Centered Health Home (PCHH) practices.

  • Program Reporting: Produce summary program reports identifying key goals, accomplishments, and outcomes, ensuring stakeholders are informed of progress and results.

  • Industry Trends: Stay abreast of new developments and industry trends related to PCMH/PCHH, clinical quality improvements, and other assigned programs to maintain cutting-edge practices.

  • QA/QI Oversight: Monitor and oversee Quality Assurance/Quality Improvement (QA/QI) programs and processes of the Federally Qualified Health Center (FQHC) as directed by the Executive Director.

  • Reporting Tools Development: Collaborate with Information Technology staff to develop and expand meaningful reporting tools related to clinical outcomes, enhancing data-driven decision-making.

  • Clinical Goals Achievement: Work with clinical teams to meet Health Resources and Services Administration (HRSA) and PCMH/PCHH identified clinical goals and outcomes, ensuring compliance and excellence.

  • Care Coordination: Coordinate care with the treatment team and create action plans for clients based on their physical and mental health needs, promoting holistic care.

  • Professional Development: Attend and participate in professional work groups to stay updated on new trends and innovations, fostering continuous learning and improvement.

  • Organizational Goals: Demonstrate interest in and support for the long-term and short-term goals and objectives of the organization, contributing to its strategic vision.

  • Team Collaboration: Work collaboratively with team members to further the vision and mission of the healthcare center, ensuring a unified approach to patient care.

  • Communication: Maintain an atmosphere of open communication with staff related to clinical trends and goals through scheduled staff meetings and frequent interactions with clinical teams.

  • Infectious Disease Control: Oversee infectious disease control, client hygiene, and related health issues, ensuring a safe and healthy environment for clients and staff.

  • Health Education: Provide disease prevention, risk reduction, and health education as needed, empowering clients with knowledge and tools for better health outcomes.

  • Consultation: Consult with physicians or advanced practice providers as necessary, facilitating expert medical advice and guidance.

  • Timely Documentation: Complete all required documentation in a timely manner, ensuring compliance with regulatory standards and organizational policies.

Education and/or Experience Qualifications:

  • Current Registered Nurse licensed in the state of practice.

  • Willingness to become licensed in additional states.

  • BCLS certification required; ACLS certification preferred.

Additional Qualifications:

  • Strong communication skills.

  • Good judgment and discernment in dealing with people.

  • Ability to work with information confidentially.

  • Thorough knowledge of nursing principles and practices in mental health and substance abuse services.

  • Capability to communicate ideas clearly and effectively to produce clear and concise reports.
