The Licensed Practical Nurse participates in planning, providing, and evaluating patient care at the appropriate skill level. They have a duty to provide a standard of care that meets or exceeds that of a reasonably competent and knowledgeable Licensed Practical Nurse. The LPN performs duties within their scope of practice delegated by a physician or ARNP. The LPN will report to the PSU Clinical Team Manager.

Position in this function oganizes the clinical environment in patient care situations. Support includes but is not limited to assisting the ARNP, including those skills listed under Job Functions below along with various other procedures as directed by the ARNP. The LPN completes FOC screenings and documents those results on the encounter form. The LPN should communicate data to the ARNP or other specified members of the clinical team within the provider’s office as needed. The LPN delivers quality customer service. Ensures policy and procedures are followed including infection control, privacy and confidentiality. This position performs all duties within the scope of an LPN, including Focus on Care screenings, phlebotomy, and collection of vital signs.

Primary Responsibilities:

  • Reviews the patient history in ePRG for contraindications to FOC screening procedures

  • Rooms patients according to company standards

  • Performs all nursing care duties within the scope of a Licensed Practical Nurse, including DPN testing, Quantaflo testing, spirometry testing, administration of the PHQ9, DRE examinations, phlebotomy, administration of FOBT/FIT tests, scheduling mammograms or colonoscopies as appropriate, and schedules patient return visit to provider’s office

  • Organizes, cleans and stocks assigned FOC equipment and lab supplies as needed to bring with him/her to all field visits

  • Communicates patient concerns, inability to collect FOC tests, and observed patient issues to the WellMed ARNP if present, or the delegated clinical representative in the provider’s office at the time of the patient screening

  • Responsible for completing FOC skills assessments of providers as needed

  • Completing all patient packets and testing procedures in a timely manner and ensuring that those results are submitted to the proper person within the PSU and a copy is left with the delegate at the provider’s office

  • Performs all other related duties as assigned

Required Qualifications:

  • High school graduate or GED equivalent

  • Current Practical Nurse License in applicable state

  • Current driver’s license and operating vehicle within the applicable state to provide transportation of equipment and self to and from various provider’s offices

  • LVN Licensure in TX

  • 2+ years of related LPN clinical experience

  • Working knowledge of medical terminology and ICD-9 and CPT coding

  • Computer literacy required with ability to complete basic EXCEL spreadsheets

  • Proven aility to react calmly and effectively in emergency situations

  • Proven good communication and customer service skills both in person and on the phone

Preferred Qualification:

  • 3+ years of LPN experience in a medical setting

Physical & Mental Requirements:

  • Ability to lift up to 100 pounds

  • Ability to push or pull heavy objects using up to 300 pounds of force

  • Ability to stand for extended periods of time

  • Ability to use fine motor skills to operate equipment and/or machinery

  • Ability to properly drive and operate a company vehicle

  • Ability to receive and comprehend instructions verbally and/or in writing

  • Ability to use logical reasoning for simple and complex problem solving

  • Ability to travel in a vehicle for extended periods of time

  • Occasionally requires exposure to communicable diseases or bodily fluids
